Work Permit checklist

This is the official checklist used by the Canadian Visa Application Center (VFS) in Tunis. You can download the forms visa CIC website (http://www.cic.gc.ca/) but you need to use the last update of Internet Explorer and Adobe Reader

1/ Original and copy of passport (bio page) with a validity of minimum 6 months including at least one blank page

2/ Application for Work Permit Made Outside of Canada form (IMM 1295), including 2D barcode page (to be filled online)

3/ 2 photos

4/ Family Information Form (IMM 5645)

5/ Schedule 1 Form (IMM 5257)

6/ Processing and biometric fees, if applicable

7/ Use of Representative form (IMM 5476 form), if applicable

8/ VAC Client Consent Form

9/ Job offer or internship letter or contract from the prospective employer giving job title, wages and working conditions

10/ Proof of Scholarship or support

11/ Copy of the Labour Market Impact Assessment (LMIA), or Offer of employment to a foreign national exempt from LMIA [IMM5802] (except Open Work permit);

12/ If you claim to be exempt from a LMIA you need to submit the proof for it. For further information you can check the following website page: http://www.cic.gc.ca/english/resources/ ... /index.asp

13/ Copy of the CAQ, if applicable (not compulsory for internships)

14/ Diplomas and transcripts, original and copies( translated in French or English)

15/ Employment certificate

16/ Police record Certificate “Bulletin n. 3”

17/ Marriage certificate or Statutory Declaration of Common-law Union form (IMM 5409) if applicable

Please note that your application must be complete at the time of the initial submission. You cannot submit additional document after the initial submission unless they have been expressly requested by the visa office. These documents must be accompanied by a copy of the request from the visa section.
