Temporary Resident Visa checklist

This is the official checklist used by the Canadian Visa Application Center (VFS) in Tunis. You can download the forms visa CIC website (http://www.cic.gc.ca/) but you need to use the last update of Internet Explorer and Adobe Reader.

You need to put in order your documents according to the checklist :

1/ Original and copy of passport (bio page) with a validity of minimum 6 months including at least one blank page

2/ 2 photo

3/ Temporary Resident Visa application form (IMM 5257), including barcode page (it must be filled in online)

4/ Family Information Form (IMM 5645)

5/ Schedule 1 Form (IMM 5257)

6/ Processing and biometric fees, if applicable

7/ Use of Representative form (IMM 5476 form), if applicable

8/ Consent form at the VAC

9/ Minor travelling alone or with one parent : Custody letter or consent form signed by the parent who is not travelling or parental authorization signed and certified by both parents or legal guardian, if applicable

10/ Copies and originals of the Employment letter and annual leave authorization or study certificate, according to the case

11/ Proof of funds for the applicant, such as bank statements and/or payslips or proof of pension entitlements

12/ Family documents such as birth certificate, marriage certificate or Statutory Declaration of Common-law Union form (IMM 5409), if applicable

13/ Invitation letter from Canada and copy of the document of the inviter (copy of the passport for Canadian citizen, or permanent resident card or work or study permit) and/or mission order, hotel booking or conference registration

14/ Medical insurance for applicants older than 60 years

Copy of the study permit, transcript, letter of acceptance, just for student returning to Canada
Copy of the work permit, just for workers returning to Canada
Copy of the CAQ just for students and workers returning to Canada

Please note that your application must be complete at the time of the initial submission. You cannot submit additional document after the initial submission unless they have been expressly requested by the visa office. These documents must be accompanied by a copy of the request from the visa section.
